RN or LPN chart reviewer-Remote

Responsible for conducting clinical chart reviews on Prospective HCC charts for the purposes of identifying and submitting accurate HCC/​RxHCCdiagnosis for members.​Ensure accurate coding using Official Coding guidelines.​ Identify opportunities for improvement in documentation.​


  • Responsible for conducting clinical chart reviews for the purposes of identifying and submitting accurate documented diagnoses for members.​
  • Records findings in database completely and accurately in accordance with standard policies and procedures.​
  • May be required to identify, collect, and compile HEDIS results data for quality initiatives.​
  • Provide feedback to client to assist in improvement of provider documentation.​


  • Current RN or LPN license required
  • Medical coding certificate a plus
  • 3-5 years previous experience including medical record documentation review and coding experience required
  • In depth knowledge of Official Coding guidelines and Medicare Risk Advantage process
  • Strong analytical and problem solving skills
  • Self-motivated and ability to work independently in a remote environment

    I have searched At Home Utilization Reviewer positions for RN and I have not been successful et I have heard of nurses who are presently performing the job. Please help me I am desperate for a job as I lost my job as Administrator because closed our office without notice and I have to daughters depending on me for support. I have been a nurse in various settings and having been in solved in this aspect I know I have much experience to bring to the position.
