GENERAL SUMMARY OF DUTIES – Reviews medical record documentation to select and sequence the appropriate ICD-9-CM diagnosis, and ICD-9-CM and HCPCS procedure codes. Applies all appropriate coding guidelines and criteria for code selections. Adheres to Company and HSC Coding Compliance policies and procedures for the assignment of complete, accurate, timely, and consistent codes for diagnoses and procedures.

DUTIES INCLUDE BUT ARE NOT LIMITED TO:

  • Codes same day surgery (SDC), observation (OBS), emergency department (ED), recurring (RCR), and clinical (CLI) records, including the assignment of ICD-9-CM, E/M level, Procedure Categories , modifiers (when applicable) and HCPCS/CPT codes across multiple facilities
  • Codes an average of 6.25 outpatient surgery records per hour; or 6.25 observation records per hour; or 25 ED records per hour; or 12 ED with E/M per hour; or 37.5 RCR records per hour; or 37.5 clinical records per hour with 95% accuracy. Initiates physician queries in compliance with Company and HSC policy where appropriate
  • Reviews all official data quality standards, coding guidelines, Company policies and procedures, and clinical/medical resources to assure coding knowledge and skills remain current
  • Utilizes the complete medical record’s documentation in code assignment
  • Meets all educational requirements as stated in current Company and HSC policy

Qualifications
KNOWLEDGE, SKILLS & ABILITIES
  • Coding Technical skills- regulatory coding (ICD-9-CM, CPT-4, and APCs) and associated reimbursement knowledge
  • Analytical skills – effective evaluation, synthesis and use of information gathered
  • Organization – establishing courses of action to ensure that work is completed efficiently; proactively prioritizes assignments and keen ability to multi-task
  • Communication – communicates clearly and concisely
  • Customer orientation – establishes and maintains long-term customer relationships, building trust and respect by consistently meeting and exceeding expectations
  • Policies & Procedures – articulates knowledge and understanding of organizational policies, procedures and systems
  • PC skills – demonstrates proficiency in Microsoft Office applications and others as required
  • Quality Orientation – accomplishing tasks by considering all areas involved, no matter how small; showing concern for all aspects of the job; accurately checking processes and tasks; being watchful over a period of time
  • Work Independently – is self-supporting; not needing to rely on others to complete a job
EDUCATION – High School graduate or GED equivalent required, Associates degree in HIM/HIT
strongly preferred.
EXPERIENCE – 1 year acute care hospital outpatient coding required, a minimum of 3 years acute care
hospital outpatient coding experience strongly preferred.
CERTIFICATE/LICENSE - RHIT, RHIA, CCS, or CCA strongly preferred.
