The Care Coordinator will manage the care of members in their homes or other community settings telephonically, focusing on disease management and keeping members healthy and independent.

Primary responsibilities:

* Engaging members telephonically to coordinate services, community resources, and treatment needs.
* Delivering a holistic approach to coordinated care based on the enrollee needs
* Identifying early risk factors and conducting ongoing assessments and documenting in an electronic file.
* Creating, reviewing and revising Plan of Care plans and focusing on disease management.
* Functioning as an advocate for members and decreasing unnecessary hospitalizations.
* Collaborating with the member’s PCP to deliver and coordinate necessary services.
* Building relationships with members and their families; assisting them with proper health choices
* Ensuring cases are documented in a timely manner.

This is a field-based/telecommuting position.

Qualifications:

* Social service experience
* Geriatric population experience
* Bachelor’s degree – social work, psychology, sociology, or human services-related field
* 2+ years of field experience with geriatric population
* Bilingual (Spanish/English)
* Case management experience – preferred
